Medi-Cal - Medi-Cal is a federal and state funded health insurance program for low-income families and children, persons with disabilities, and seniors who qualify for help. Eligibility and enrollment services are provided by the Alameda County Social Services Agency.

Healthy Families Program - The Healthy Families Program provides low-cost health insurance to children up to age 19 whose family incomes are too high to qualify for Medi-Cal, but below 250% of the Federal Poverty Level ($41,500 for a family of three). Benefits include medical, dental and vision coverage. The Alliance is the Community Provider Plan (CPP) in Alameda County. If you choose the CPP, your monthly premium will be lower than if you choose another Healthy Families plan in Alameda County. The Healthy Families Program is available through the Alliance for $4-12 per child, with a maximum of $36 per household per month. There are $5 copayments for office visits, prescription medicine, and emergency room services, with a limit of $250 per family per benefit year. There are no copayments for immunizations and preventative care. The program serves children who are U.S. citizens, nationals, or eligible qualified immigrants.

Alliance Group Care - Alliance Group Care is an employer-sponsored plan that provides comprehensive health care coverage to In-Home Supportive Services (IHSS) workers in Alameda County through a contract with the Public Authority for IHSS in Alameda County. Alliance CompleteCare - Alliance CompleteCare is a Medicare Advantage Special Needs Plan for people of all ages who live in Alameda County and have Medicare and Medi-Cal (or are eligible for Medi-Cal). Alliance CompleteCare combines Medicare, Medi-Cal, and Part D drug coverage into one easy-to-use plan. Members pay $0 plan premiums, $0 deductibles, and $0 co-pays for all covered medical benefits. Personal Care Advisors help Alliance CompleteCare members get the health care services they need, when they need them. Covered benefits include preventive, emergency, and urgent health care services. Members also qualify for dental, hearing and vision care, prescription drug coverage, diabetes self-care supplies, acupuncture and chiropractic care, hospital and skilled nursing facility care, and home health care. The following services are available to our members.

bullet Personalized doctor care - Members can choose their personal Primary Care Provider (PCP) from hundreds of PCPs in the Alliance network. Our network doctors are conveniently located and many speak a variety of languages.

bullet Bilingual Member Services Representatives - Alliance has Member Services Representatives who can assist callers in several languages, including Spanish, Cantonese, Mandarin and Vietnamese. We also use telephonic interpreters available to assist callers with other language needs.

bullet Health Promotion Department Health Education offers a variety of materials and programs to assist members in making healthy changes in their lives. This support is free of charge to Alliance members and includes weight management, quit smoking, parenting, infant and child health, pregnancy and child birth, asthma and diabetes.

bullet Education classes for special health needs, such as asthma and diabetes, which provide free education materials.

bullet Interpreter services available at no cost to you or your provider for all services covered by the Alliance.

bullet Quarterly newsletter translated into several member languages.
